Make "crosshair by health" take armor into account
The "crosshair by health" option would be more useful if it took armor into account for its calculations (like in id Tech 3 games). For example, a player with 100 health and 100 armor should see a green crosshair (just like when having 200 health), instead of a white one. The calculations should use the armor protection rate defined by the server, falling back to the default value (0.6) in case it is not sent, for compatibility with old servers.
