Redesign phone number entering fields

  • The new fields are easier to use
  • The country code prefix is visible before the phone number part
  • Specifically check that the phone number field is not empty
  • Don't allow zero as a first number in the phone number field

Fixes #402 (closed)

The country selection dropdown items could use some visual care too, but let's do that separately.
