[Feature Suggestion] Consider the Incapacitated status effect for flanking calculation

Tokens with an assigned Incapacitated status effect cannot give Flanking to allies following the rule in the DMG. Actors can be incapacitated when not at zero hp (e.g., when they are paralysed).
