Extra chat card on saving throw

I've been testing out an ability with secondary damage that is reduced by a saving throw. For some reason, the saving throw is not getting condensed to the large card. Below is an image to help explain what I mean. The Con Saving Throw chat card from Vishara is what I am talking about. As far as I can tell that shouldn't be showing up, but it shows both for the GM and players. I've also attached my current settings. I've tried every setting that I can think of that might effect this with no luck. I also tested without any other modules enabled to ensure it wasn't being caused by another module.
