[FIX] Increase the uniqueness of TxOutItem stored in bifrost

When I am testing ragnarok , due to the test data , the network generated multiple exactly the same txout item. As a result of that txout item stored in bifrost get override by each other , this will increase the uniqueness of txout item by adding a index to represent the position
