Remove of more than 9000 sequential keys in the same transactio ncause index corruption

The remove of more than 9000 sequential keys cause some empty pages left in the index because the current index logic do not handle properly the merge of pages that are across "gran-parent" nodes in a single transaction, this left over empty pages make some range operations fails because try to read a keys in empty page.
