update CI runners to have opentelemetry >=1.16.0

Some CI runners have older versions of opentelemetry (the MacOS runners have 1.15.0 and the cmake-with-locations-... runners have 1.11.0). These should be updated to at least 1.16.0 to resolve issues related to API <-> SDK tracers/loggers.
