Add a new Troubleshooting section to the documentation

Troubleshooting should be its own section with actionable solutions to common problems during install or runtime.

Tasks:

• Create a docs/troubleshooting/ folder
• Add known issues: bootstrap, provisioning, failed installs
• Encourage contributors to report/pull fixes from issues
