fix exact

* module/hoot/stdlib.scm ($decode-f64): Adjust arguments of `$lsh'.
($f64->exact): Use correct variables for raw vs. processed data; adjust
the exponent bias to account for returning the significand as an integer
rather than a float.
* test/test-numeric.scm: Reenable float-fraction comparison and `exact'
tests.
