Form templates

Templates will be useful when creating a new form. Templates examples: person profile, event, etc.

Scenario:

• user wants to create a new form
• user selects "New form" and opens "new form" page
• the page has a dropdown with predefined templates
• the page has name and description fields to create a form from scratch
