Add issue title to default template

The issue description is pre-filled when someone opens a new issue, but a default title is not automatically filled. In this case, we do want a consistent title to make these requests easier to surface:

Default issue title: FirstName LastName (FAS: MyFasId) - EmployerName - EventName
