KVM: x86: Backport SMM related fixes

This backports series of SMM fixes, including fix for windows guests crashing when using secure boot on intel due to invalid VMX state.

Tested: Tested with KVM unit tests in a guest which run a RHEL 9 kernel with those patches applied.
