Add admin action(s): "Set reason"

Add menu items to the admin action menu based on existing reason values in the DB. E.g. if reasons found in the DB are "spam" and "TOS", menu would have:

Set selected IPs' reason to "spam"
Set selected IPs' reason to "TOS violation"
