Create additional options for Export Results to include survey information as headers

First step - take no more than an hour to estimate...

The goal is to support better human and AI usability of the exported data from a survey. We will start only by updating the CSV export, though we may want to also include this information optionally in our results page.

We're including key information from the Survey in the header of the CSV as shown below...
