Creation of page: LCCWG/Affiliation Model Subcommittee

Hi LCCWG,

Last week, I shared an idea in the Telegram Chat also on creating a subcommittee page in the OSMF website like what we did in with the Moderation Subcom (https://osmfoundation.org/wiki/Local_Chapters_and_Communities_Working_Group/Moderation_Subcommittee) for documentation and recognition of volunteers.
