Support SAML auth with AnyConnect

Over at dlenski/openconnect #143 on Github, we've got two reports of "AAD SAML" auth used with AnyConnect (Azure Open Directory).

Need logs or access to an AnyConnect server that uses SAML to know how to support this. #45 may be related.
