Item doesn't appear or exist in Samarys Ancestral Tomb

Went through the quest went to tomb, checked ENTIRE tomb, Item doesn't appear or exist in Samarys Ancestral Tomb.

Item is not on monsters, in any containers, or on laying anywhere in vicinity. Tried creating a new save - tried merging files. Using OpenMW version 0.49.0 Revision: a24fa3ea51.

Is there any way to add the item through console to see if something else is awry?

Checked in the script found out the item id: marksmans_eye_item no idea why it doesn't spawn in the tomb.. it let me place it in my bag from console.
