Failed to start Repartition Root Disk

This is a fresh install on my OP6 (enchilada). Followed the instructions from the wiki. I also am having a hard time getting pmos to boot. Here is a picture of what I see on the first boot after flashing the images

I'm starting to think I might have a defective phone but let me know what you guys think
