(bug): The "Report an issue" form doesn't work.

Summary

Clicking "SUBMIT" produces an error message. See screenshot below.

NOTE: Canary mode is turned on.

Steps to reproduce

  1. Go to abbeyroad.minds.com/issues/report
  2. Type "test" into all 4 input fields.
  3. Click "SUBMIT".

Windows 10 / Chrome (Version 73.0.3683.86)

What is the current bug behavior?

The "Report an issue" form doesn't work. Clicking "SUBMIT" produces an error message.

What is the expected correct behavior?

It should actually submit the form and display a confirmation message.
