Syncing schemas for database targets

One of several common tasks for database-type targets that's currently missing from the SDK is the ability to create tables based on schema messages, and handle schema changes. Targets seem to handle this a number of different ways, and having some convention in the SDK is valuable on top of the obvious value of accelerating development of database targets.
