Scheme-ify (and improve) Measure_grouping_engraver

Make Global_context::add_moment_to_process available to scheme and, as an application, re-implement the Measure_grouping_engraver in Scheme.

While at it, make sure the measure grouping engraver does not miss moments at which no musical event happens.
