LibreHealth Toolkit: Create a mockup of the patient dashboard

NOTE: This dashboard is going to be viewed by a doctor, not a patient.

Your task is to create a wireframe of the patient dashboard, such that it covers the following features:

• patient demographics: name, age, date of birth, sex, gender, and any other administrator customized person attributes
• list of last 5 visits
• active lists - medication, problems
• patient image
• Enrolled program(s)
• Charts of observations
• next appointment

This must be original work and must be in your own words. Any assets you use must be Creative Commons and attributed accordingly, or be your own original work. If your work is not original, then in a separate document, list the sources plus the license.
