Rename LibreHealth EHR Patient Portal Directory

You must have completed the Become a part of the community task prior to completing any others!

First clone the code from GitHub

Rename the LibreHealth EHR/patients directory to LibreHealth EHR/patient_portal.

Correct all file and URL's in the code base to reflect the new directory name.

Submit your work as a pull request to LibreHealth EHR GitHub.
