[Azure DevOps | report a new issue] The bug report form is empty

1️⃣ Impacted version: 

• Azure: Cloud & Server
• Squash : All versions starting from v11.0.0.RC4; RC3 and previous versions work correctly.

2️⃣ Steps to reproduce:

• For a project hosted on Azure DevOps Cloud, attempt to report a bug from Squash.

3️⃣ Observed behaviour: 

The bug report popup is empty, and the browser inspector shows an error.

Even if the form is displayed, the anomaly report no longer works for most work items:

4️⃣ Workaround: I haven’t found a solution for Azure Server, but for Azure Cloud, you need to:

• Switch the project to a custom process, based on the Agile model.
• Create a test case and a test suite in Azure (Work items > New work item)
• In the process configuration for test cases and test suites, add a 'Severity' field.
