replace PS/PDF flow diagram PNG with SVG

Using a vector graphic instead of a low resolution PNG removes pixelation.

The source of this graph is unknown. So this commit attempts to recreate it. The
graph is intended to be much the same, except a sans serif font is used for
better screen readability.
