Docs survey on docs.gitlab.com

Axil, we would like to post a survey for GitLab docs users. We can ask for a banner to be implemented on one or two pages. That's not a lot.

The intercept would provide us with more responses. It requires code to be added. We don't do this on Gitlab proper because of the user tracking concerns (telemetry). The docs are simply the docs.

What are your thoughts?
