Allow self-managed customers to upgrade their subscription in the portal

Overview

There isn't currently a way self-managed for customers to upgrade their subscription in the portal, either mid-term or during the renewal process. This causes frustration and missed revenue.

Proposal

1. Allow self-managed customers to upgrade their tier while renewing
2. Allow self-managed customers to upgrade mid-term

Consider leveraging the flow that exists for .com upgrades.
