add issues template

This MR implements an issues template that can be set for a project.

I wanted to have green tests as well, but after 16 unrelated test failures I'm fed up with it.

What's with these pending tests?!

only 3 public runners are active. It takes several hours before the tests start and I couldn't get 2 of them to work at all because they fail randomly on completly unrelated code or run into timeouts.
That's why I finally gave up on them :/
