Improve index usage of queries to approval_merge_request_rules and approval_project_rules

### Background
As @acroitor suggested in https://gitlab.com/gitlab-org/gitlab/-/merge_requests/142169#note_1760515789, we can improve index usage of some queries.
### Proposal
Look into if adding a new index or updating existing indexes can help improve queries further.
