When you add an issue to an epic that is already in the epic, you get erroneous system notes

In the situation shown by the screenshot below, gitlab-ee#6903 was already part of epic &272 (closed).

I pasted the issue link again into the epic, and that generated the two system notes shown below. That should not happen. No system notes should be added in this case.
