[Sessions Index] Update Session Information Format

Problem

Update the existing Session items to the new format to include Created date and Last update message.. as well as including the Project name

Solution

Update the Sessions items to each have the following format:

{Status icon} {Project Name} / {Flow name} - {Session ID}
              Created {# minutes ago}
              {Last update message, single line, truncated} • {# minutes ago}
