Remove preserve_markdown feature flag and associated code

Remove the preserve_markdown feature flag and associated code, following the unfortunately tumultuous journey it had with blockers requiring somewhat uncapped R&D to solve (sourcepos fidelity), and lack of a clear value proposition.
