Duo Code Review failure after requesting a review automatically

Please let me know if there's a better place to post this, but I've noticed that since we've enabled auto GitLab Duo review on the https://gitlab.com/gitlab-com/gl-infra namespace, about a week to 10 days ago, every since MR that I've created has the following error posted:

Steps to reproduce

1. Enable Automatic reviews from GitLab Duo for a project
2. Open a MR and a review from GitLab Duo will be requested
3. Duo Code Review will fail with a generic error message

What is the current bug behavior?

Duo posts a generic error message and review is never completed.

What is the expected correct behavior?

Duo actually reviews the MR and add comments with suggestions.
