Implement Duo Context Exclusion in MCP Tools

Right now, the Duo Agent tools implement Duo Context Exclusion

• get_commit_diff
• list_merge_request_diffs
• get_repository_file
• list_repository_tree
• read_files
• write_file

We need to modify the equivalent MCP tools to support Duo Context Exclusion as well. This includes any tool that touches repository file or diff data.

Since these are implemented in the Gitlab monolith they would probably want to make use of Ai::FileExclusionService
