[VS Code] Give self-managed users the option to select OAuth when running the "GitLab: Authenticate" command

Define new configuration properties to configure an instance URL/client ID mapping. If the user's user/workspace setting contains the instance URL pre-populate the client ID field and simply prompt them to confirm the instance URL.
