Notification emails get lost/are not delivered for MRs and Issues, getting unsubscribed from Issues.

@dgruzd and I recently noticed that we get silently unsubscribed from issues or we do not get notifications for Issues/MRs we are 100% sure we subscribed to/are subscribed to.

In my case, I did not get notifications for these two issues:

• #525855
• gitlab-com/gl-infra/delivery#20909 (closed) - in case of this one, I even got unsubscribed at some point.
