Remove prompt migration feature flags

We recently enabled prompt migration feature flags for 6 features:

prompt_migration_epic_reader
prompt_migration_explain_code
prompt_migration_fix_code
prompt_migration_issue_reader
prompt_migration_refactor_code
prompt_migration_write_tests

Ask: Now that these flags are enabled, remove the FF and the associated code from the codebase
