Bug in creating a new child epic after creating a new child issue

Summary

When creating a new child epic in the hierarchy widget after creating a new child issue, the new "epic" is created as an issue in the same location as the previously created issue.

Steps to reproduce

1. Create a new work item epic
2. Using the hierarchy widget, create a new child issue (select a project)
3. Using the hierarchy widget, create a new child epic
   1. Notice there is no Group selector presented
   2. Notice the resulting record is an issue record created in the same location as the child issue you previously created
