[Bug] Insert suggestion in MR includes diff "+"

Noticed on .com in !146072 (comment 1842547291)

bug_insert_suggestions_in_discussion

1. Start a discussion on a "+" part of a diff.
2. In the MR Overview tab, reply to that discussion with an "Insert Suggestion"
3. The new suggestion will include the "+" of the diff.
