Approval rules for deployments to protected environments require that all users approve instead of one.

Steps to reproduce

1. Create a protected environment
2. Add Maintainers role to the Approvers
3. Add multiple individuals to the Approvers

Deployment approval now requires that every individual approves instead of one.
