Additional LDAP login form custmomization

Problem to solve

The LDAP login form can only be modified with the label configuration in the gitlab.rb. Add additional fields (perhaps configured via the admin). To adjust the title and username labels.

Example:

main:
  label: 'LDAP AD001'

The label setting will adjust both the title and the username label.

Using the service name/label for the title, but specifically asking for 'email address' rather than "LDAP AD001 Username". This provides additional clarification to end-users.
