[🎨 Design] Work Items -Improvements to design management discussions

Context

As we migrate design management over to work items, how might we address SUS feedback and improve usability around discussing designs?

Note: We would need to keep the scope of this small for this iteration until we devote more resources to the feature.

From a comment in the related issue:

there may be ways to simplify the experience here too. @nickleonard and I were talking about ways to potentially leverage the existing discussion threads for the design manager, which would also address some of the SuS feedback I believe.

Plan

• Discuss any overlap with current work items efforts and document proposed improvements
• Provide any designs needed here and within the larger vision of #385195 (closed)
• Address any related existing SUS issues where possible
