Required approval triggered by scan result policy has a confusing name

Proposal

When a scan result policy is triggered and an approval is required, it shows up in the UI as "Requires 1 approval from <policy name>". This generates a sentence that doesn't really make sense if your policy name describes the actual policy rather than the approvers.

I would suggest having another field in the policy for "Approver(s) name(s)" or simply automatically generating it from the configured approvers' display names.

Another alternative would be to have another sentence saying "Approval triggered by <policy name>" but that might make things complicated when it's mixed with a CODEOWNERS approval.
