Support multiple alerts for a single metric

As part of iterating on features, we want the ability to support multiple alerts for a single metric on our monitoring dashboards, this feature depends on the work from the following issue
User flow
As a user i would like to set an alert when a metric crossed a threshold, and a 2nd alert when it crossed a 2nd threshold.
e.g. a user measure error rate and set an alert for 5% and a 2nd alert for 10%
