Upload new file icon broken

What is the current bug behavior?

Upload modal doesn't show the right icon other formats. It only shows images and then a broken <img> icon for the rest of formats.

What is the expected correct behavior?

It should show image previews and then the same set of SVG icons that the file table shows.
