Insert, edit, or navigate to a footnote in the Content Editor

Footnotes are a useful feature for navigating large documents, but scrolling between the reference and definition can be disruptive. Since footnotes are effectively anchor links within a document, we can follow the same pattern in the Content Editor as editing a link.
An icon in the toolbar should allow someone to insert a new footnote link and clicking on an existing footnote reference like footnote[1] should open a popover similar to the one used to edit links. The popover should display the description inline, and let you edit the description or visit the reference.
