Docs & UX feedback: Design Management UI is hidden for first time users and documentation does not help

Problem to solve
- The Design Management UI is hidden when first creating an issue, you must save and reopen to see the options. It is non-discoverable when you wish to directly create an issue specifically for design feedback.
- The documentation shows a starting point that is not for a new issue.
- The documentation shows a starting point that is also not for an edited issue that does not yet have any designs.
- The documentation shows a starting point that is an existing issue that already has some designs.

The Design management page here attempts to walk the user through getting started with designs.
However, the first time through experience does not match the page.
- The first time through you must save and re-edit the page to see any design management controls.
- Before adding a first design, the control looks like the below - not like the web pages shows:
Proposal
- The documentation should be updated to a "new issue" starting point as soon as possible to reflect how to discover this functionality as-is as of the writing of this issue.
- Some type of ability to: a) know about the designs feature and b) add designs should be present in the "Create Issue" UI.
