Add missing icon tooltips in merge request page

Per https://design.gitlab.com/product-foundations/iconography#icon-meaning:

If an icon is not accompanied by a label, provide a quick explanation for users in a tooltip.

Even with the tooltips, let's make sure that icons have attributes or hidden text that is announced by screenreaders.
