Attempting to create new issue gives error "500 Cannot create issue getting Whoops, something went wrong on our end"

When anyone on our team (https://gitlab.com/petsc) attempts to create a new issue they can fill in the information, but after pressing the Submit button a new message “500 Whoops, something went wrong on our end.” comes up so we are unable to make new issues.

This started over a week ago; before that it worked flawlessly for everyone.

A team member who could not create an issue in our project tried to create an issue on another project and it worked fine.

Searches as to the cause of the problem found nothing useful. We have posted about this on the Gitlab forum over a week ago and have received no response.
