Changes to issue description by different users attributed to same user if done within five minutes of each other

Summary

If two different users edit the same issue description within five minutes of each other, both of the edits are attributed to one user.

Steps to reproduce

1. Open an issue.
2. Edit the issue description.
3. Edit the issue description again from a different account within five minutes of the last edit.

What is the current bug behavior?

Both of the edits appear as a single entry in the issue notes/history/comment section, attributed to the user who edited it last, like so:

[user] changed the description 2 times within 5 minutes · just now

What is the expected correct behavior?

The issue notes/history/comment section should contain an accurate log of who edited the issue description and when.
